Assistantship Induction form
At the start of the Assistantship, you’re required to meet with your clinical supervisor to discuss and agree objectives for the Assistantship period. During this meeting, you need to complete the Assistantship Induction form. This form requires sign-off from your supervisor.
Immediate Life Support course
Attendance at the Immediate Life Support course is compulsory. Your attendance will be recorded in OpenCampus by the Hull York Medical School Programme Delivery team.
Case Based Discussion form
During the 8 week Assistantship period, you’re required to complete two Case Based Discussions (CBD). These CBD forms are completed on the OpenCampus app and follow the same workflow as the CBD you completed throughout Year 5 of the programme.
You must complete both CBD tasks by the end of week 6.
Team Assessment of Behaviour form
During the Assistantship, you’re required to complete a 360-degree Team Assessment of Behaviour (TAB) based around professional behaviours. In the second or third last week of the Assistantship period you need to complete 3 TAB forms on the OpenCampus app. One of these forms should be completed by a supervising consultant, one by a Foundation doctor and one by a senior nurse (ideally at band 6 or 7) that you have worked with.
All three TAB forms should be completed before the Assistantship review meeting takes place.
In the final week before the Assistantship review meeting, you must complete a reflection about your Assistantship experience. This form is completed via the online system: https://opencampus.hyms.ac.uk.
In the final week of the Assistantship, you need to complete an Assistantship Review form with your Clinical Supervisor. This form requires sign-off from your supervisor.
