OpenCampus Phase II Portfolio

This article will explain how to complete the requirements of the Phase II portfolio in OpenCampus.

The OpenCampus portfolio provides an ongoing record of your progress through the school. It provides an important source of developmental (formative) feedback as well as a record of completed work and achieved grades. The Open Campus Portfolio should be the focus of a discussion with your Education Supervisor at the beginning and end of each new placement.

The portfolio must be kept up to date. You will need to work with your educational and clinical supervisors to ensure that all portfolio requirements are completed in a timely fashion.

It is important to remember that satisfactory completion of the OpenCampus Portfolio is a mandatory requirement for entry into the end of year summative assessments (as outlined in the Code of Practice on Assessment and Examination).

For each placement in Phase II of the MB BS programme, you’re required to complete a number of portfolio activities. Full details on the Phase II portfolio requirements can be found in the Phase II handbook available on Blackboard.

In exceptional circumstances, it may be possible for the programme delivery team to reopen web-based portfolio activities. Please contact phase2@hyms.ac.uk directly about this.

Accessing your OpenCampus portfolio

You will complete your portfolio in OpenCampus via the online system and the mobile app.

Access portfolio activities (forms) via the online system

Follow these steps to access your portfolio via the online system:

  1. Log in to OpenCampus (https://opencampus.hyms.ac.uk) with your HYMS account (email address and password).
  2. From the OpenCampus menu, select Portfolio.
  3. You will now see your portfolio homepage. The portfolio is broken down by placement. For example, a year 3 student will see the following tabbed menu: Placement 1Placement 2Placement 3 and Placement 4. A year 4 student will see Placement 5, Placement 6, Placement 7 and Placement 8.Screenshot of a students portfolio within OpenCampus
  4. Under each tab you will see a list of the portfolio activities (forms) for that placement. You can click on each item to see how many forms you’re required to complete and how many you have completed. You will also be able to view the details of the forms you have completed.

Portfolio forms completed in the online system

You will complete the following portfolio activities (forms) in the online system:

Clerkings

Before starting a clerking, you must make sure that you have obtained the patient’s consent, you must record this consent in the hospital or GP notes. When completing a Clerking form in OpenCampus, you are required to confirm that patient consent was given. 

Follow these steps to complete a Clerking form in OpenCampus:

  1. On the Portfolio page, select the Clerking forms item underneath the relevant placement tab.
  2. Click edit next to the form you would like to complete. This will open the Clerking form.Screenshot of the Phase II portfolio
  3. Complete all the form fields as required.
  4. Once you have completed the form, select submit. 
  5. You will see a warning message: ‘this action cannot be undone and you cannot change the form later. Do you want to submit?’. Click OK to submit the form.
  6. You will now be redirected to the portfolio homepage. A message will appear at the top of the page, stating that a form has been successfully updated.
  7. If you navigate back to the form you have just completed, you will see the status of this form is now set to ‘action required by the educational supervisor’.
  8. Your educational supervisor will review the clerking and a provide grade and feedback.  Once they have done this, the form status will update to ‘complete’ and the progress count for this portfolio activity will update.

    This form will only be marked as complete when both parties have completed their actions. It’s important to submit your clerkings in advance of the placement deadline to ensure that your Educational Supervisor has sufficient time to provide you with feedback.

  9. Once complete, you can access the grade and feedback from your Educational Supervisor by viewing the completed form.

Your Educational Supervisor will discuss these Clerkings with you in your clinical placement review meeting.

Student reflections

Further information about the reflections you’re required to complete as part of your portfolio, including what is expected in terms of length and number of reflections, can be found in the Ethics and Professionalism area of the Year 3 and Year 4 courses on Blackboard.

Follow these steps to complete a Student Reflection form in the online system:

  1. On the Portfolio page, select the Reflection item underneath the relevant placement tab.
  2. Click edit next to the form you would like to complete. This will open the Reflection form.
  3. Complete all the form fields as required.
  4. Once you have completed the form, select submit. 
  5. You will see a warning message: ‘this action cannot be undone and you cannot change the form later. Do you want to submit?’. Click OK to submit the form.
  6. You will now be redirected to the portfolio homepage. A message will appear at the top of the page, stating that a form has been successfully updated.
  7. If you navigate back to the form you have just completed, you will see the status of this form is now set to ‘complete’. The progress count for this portfolio activity will also be updated.
  8. You can click on view to review submitted content.

Your Educational Supervisor will review your reflections and discuss these with you in your clinical placement review meeting.

OSLERs

Your OLSER tutor will provide you with written feedback after the examination and it’s your responsibility to upload this to OpenCampus. You will need to take a photograph of the written OLSER feedback form and upload this to the online system.

Follow these steps to upload your OLSER feedback and grades in the online system:

  1. On the Portfolio page, select the OSLER item underneath the relevant placement tab.
  2. Click edit next to the form you would like to complete. This will open the OSLER form.
  3. Complete all the form fields as required. Firstly, upload the image (photo) of your OLSER feedback form. Click Choose File and upload the feedback file to the system.Phase II OSLER form
  4. Using the drop-downs provided, enter the grades you achieved for each criteria of the OSLER: Gathering Information, Physical/Mental state examination, Problem solving and Relationship with patients.

Refer to your written OSLER feedback form to ensure you enter this information accurately.

  1. Manually enter the name of your OSLER tutor. 
  2. Once you have completed the form, select submit. 
  3. You will see a warning message: ‘this action cannot be undone and you cannot change the form later. Do you want to submit?’. Click OK to submit the form.
  4. You will now be redirected to the portfolio homepage. A message will appear at the top of the page, stating that a form has been successfully updated.
  5. If you navigate back to the form you have just completed, you will see the status of this form is now set to ‘complete’. The progress count for this portfolio activity will also be updated.
  6. You can click on view to review submitted content.

Your Educational Supervisor will review your OLSER performance and discuss this with you in your clinical placement review meeting.

Clinical placement review form

At the end of each placement you’re required to complete a Clinical Placement Review form with your Educational Supervisor. This form is initiated and completed by your Educational Supervisor.

In your portfolio, within the clinical placement review item you will see a form with a status of ‘action required by Educational Supervisor’.

Phase II Clinical Placement Review

Once your clinical placement review meeting has taken place, and your Educational Supervisor has completed this form, it will be marked as complete in your portfolio. On completion the progress count for this portfolio activity will be updated.

Follow these steps to review the feedback provided by your Educational Supervisor on this form:

Please note: this form will include links to the case based discussion, reflection, clerking and OSLER forms completed as part of this placement.

  1. On the Portfolio page, select the Clinical Placement Review item underneath the relevant placement tab.
  2. Click view next to the form you would like to review. This will open the completed Clinical Placement Review form.

You will receive an OpenCampus generated email reminding you to read the completed Clinical Placement Review form and advise the programme delivery team (phase2@hyms.ac.uk) of any queries you have.

Portfolio forms completed using the OpenCampus app

You will complete the following activities in the OpenCampus app (these activities are completed in a clinical setting):

Case based discussions

Case-based discussions are an opportunity to discuss a patient you have seen in secondary care and receive immediate feedback. Any qualified doctor in the hospital can discuss the case with you and then complete the CBD form on the OpenCampus app. You should present your case to a clinician and discuss the key aspects of the case.

Follow these steps to complete a CBD form:

  1. Open the OpenCampus app on your mobile device. From the app homepage menu, select the Tasks tab.
  2. You will see a list of forms you are required to complete. Select the Case Based Discussion form from ‘my tasks’ and then click Start to open the form.

If you can’t see the form in the list, select ‘show all tasks’ to make all the forms visible.

  1. Before handing your device over to the clinician (assessor) for feedback, enter a short description of the case discussed.
  2. Hand your device to the assessor. Your assessor will provide you with a grade for the following criteria: Practising holistically, Data gathering and interpretation, Making a diagnosis/decisions, Clinical management, Maintaining an ethical app and Overall assessment.
  3. To complete the form, the assessor will enter their name, email address and signature before submitting. The form will then sync with the online system and update the status of the form to complete (green tick). The progress count for this portfolio activity will also be updated.
  4. You will be able to review the grades and feedback you received in the online system.  On the Portfolio page, select the Case Based Discussion item underneath the relevant placement tab.
  5. Click view next to the form you would like to review. This will open the completed Case Based Discussion form.
Clinical supervisor feedback

Clinical supervisor feedback forms should be completed in secondary care when ‘opportunistic’ or ‘real time’ feedback opportunities present themselves. Feedback can be received on any observed clinical activity.

Follow these steps to complete a Clinical Supervisor Feedback form:

  1. Open the OpenCampus app on your mobile device. From the app homepage menu, select the Tasks tab.
  2. You will see a list of forms you are required to complete. Select the Clinical Supervisor Feedback form from ‘my tasks’ and then click Start to open the form.

If you can’t see the form in the list, select ‘show all tasks’ to make all the forms visible.

  1. Before handing your device over to the clinician (assessor) for feedback, enter the activity the clinician observed you performing.
  2. Hand your device to the assessor. Your assessor will provide you with some feedback on your performance – the areas in which you perform well and areas where you can further improve.
  3. To complete the form, the assessor will enter their name, role, email address and signature before submitting. The form will then sync with the online system and update the status of the form to complete (green tick). The progress count for this portfolio activity will also be updated.
  4. You will be able to review the grades and feedback, you received in the online system.  On the Portfolio page, select the Clinical Supervisor Feedback item underneath the relevant placement tab.
  5. Click view next to the form you would like to review. This will open the completed Clinical Supervisor Feedback form.

Your Educational Supervisor will review your Clinical Supervisor Feedback forms and discuss these with you in your clinical placement review meeting.
