This article will explain how to complete the requirements of the Phase III portfolio in OpenCampus.
For each placement in Phase III of the MB BS programme, you’re required to complete a number of portfolio activities. Full details on the Phase III portfolio requirements can be found in the Phase III handbook available on Blackboard.
Accessing your OpenCampus portfolio
You will complete your portfolio in OpenCampus via the online system and the mobile app.
Follow these steps to access your portfolio via the online system:
- Log in to OpenCampus (https://opencampus.hyms.ac.uk) with your HYMS account (hyxxxx@hyms.ac.uk and HYMS password).
- From the OpenCampus menu, select Portfolio.
- You will now see your portfolio homepage. The portfolio is broken down by placement. Click on / open in a new tab images to enlarge them.
- Under each tab you will see a list of the portfolio activities (forms) for that placement. You can click on each item to see how many forms you’re required to complete and how many you have completed. You will also be able to view the details of the forms you have completed.

Portfolio forms completed in the online system
You will complete the following portfolio activities (forms) in the online system.
Your OLSER tutor will provide you with written feedback after the examination and it’s your responsibility to upload this to OpenCampus. You will need to take a photograph of the written OLSER feedback form and upload this.
Follow these steps to upload your OLSER feedback and grades to the online system:
- On the portfolio page, select the OSLER item underneath the relevant placement tab.
- Click edit next to the form you would like to complete. This will open the OSLER form.
- Click choose file and select the image (photo) of your OLSER feedback form from your device. Select upload.
- Then complete all the form fields as required. Using the drop-downs provided, enter the grades you achieved for each criteria of the OSLER.
- Manually enter the name of your OSLER tutor.
- Once you have completed the form, select submit.
- You will see a warning message; ‘this action cannot be undone and you cannot change the form later. Do you want to submit?’. Click on OK to submit the form.
- You will now be redirected to the portfolio homepage. A message will appear at the top of the page, stating that a form has been successfully updated.
- Navigate back to the form you have just completed. You will see the status of this form is now set to complete. The progress count for this portfolio activity will also be updated.
- You can click on view to review submitted content.
Your educational supervisor will review your OLSER performance and discuss this with you in your clinical placement review meeting.
Interprofessional learning enables students to understand and respect the multidisciplinary perspective of patient care and how it relates to the role of doctors.
Follow these steps to complete an interprofessional education (IPE) case template form in the online system:
- On the portfolio page, select the inter-professional education case templates item underneath the relevant placement tab.
- Click edit next to the form you would like to complete. This will open the IPE form.
- Complete all the form fields as required.
- Once you have completed the form, select submit.
- You will see a warning message; ‘this action cannot be undone and you cannot change the form later. Do you want to submit?’. Click on OK to submit the form.
- You will now be redirected to the portfolio homepage. A message will appear at the top of the page stating that a form has been successfully updated.
- Navigate back to the form you have just completed. You will see the status of this form is now set to complete. The progress count for this portfolio activity will also be updated.
- You can click on view to review submitted content.
Your educational supervisor will review your IPE forms and discuss this with you in your clinical placement review meeting
In each placement you’re required to complete a number of reflection forms. In your final placement, you’re also required to complete an end of year self-appraisal form.
Follow these steps to complete a reflections on experience form in the online system:
- On the portfolio page, select the reflections on experience item underneath the relevant placement tab.
- Click edit next to the form you would like to complete. This will open the form.
- Complete all the form fields as required.
- Once you have completed the form, select submit.
- You will see a warning message; ‘this action cannot be undone and you cannot change the form later. Do you want to submit?’. Click on OK to submit the form.
- You will now be redirected to the portfolio homepage. A message will appear at the top of the page stating that a form has been successfully updated.
- Navigate back to the form you have just completed. You will see the status of this form is now set to complete. The progress count for this portfolio activity will also be updated.
- You can click on view to review submitted content.
Your educational supervisor will review your reflections and discuss these with you in your clinical placement review meeting.
In each placement you’re required to complete a number of practice prescription activities with your allocated pharmacist. During the face-to-face discussion, the pharmacist will complete and sign a paper based assessment form. It’s your responsibility to upload this to OpenCampus. You will need to take a photograph of the written prescribing form and upload this to the online system.
Follow these steps to upload your pharmacist assessed prescribing activity form to the online system:
- On the portfolio page, select the pharmacist assessed formative prescribing forms item underneath the relevant placement tab.
- Click on edit next to the form you would like to complete. This will open the form.
- Click choose file and select the image (photo) of your prescribing form from your device. Select upload.
- Complete all the form fields as required. Manually enter the name of the pharmacist.
- Once you have completed the form, select submit.
- You will see a warning message; ‘this action cannot be undone and you cannot change the form later. Do you want to submit?’. Click on OK to submit the form.
- You will now be redirected to the portfolio homepage. A message will appear at the top of the page stating that a form has been successfully updated.
- Navigate back to the form you have just completed. You will see the status of this form is now set to complete. The progress count for this portfolio activity will also be updated.
- You can click on view to review submitted content.
Your educational supervisor will review your forms and discuss this with you in your clinical placement review meeting
At the end of each placement you’re required to complete a clinical placement review form with your educational supervisor. This form is initiated and completed by your educational supervisor.
In your portfolio, within the clinical placement review item you will see a form with a status of action required by educational supervisor.
The form will be marked as complete once your clinical placement review meeting has taken place and your educational supervisor has completed the form. On completion the progress count for this portfolio activity will be updated.
Follow these steps to review the feedback on this form once it has been provided by your educational supervisor:
- On the portfolio page, select the clinical placement review item underneath the relevant placement tab.
- Click on view next to the form you would like to review. This will open the completed form.
Portfolio forms completed using the OpenCampus app
You will complete the following activities in the OpenCampus app (these activities are completed in a clinical setting).
Case based discussions are an opportunity to discuss a patient you have seen in secondary care and receive immediate feedback. You should present your case to a clinician and discuss the key aspects of the case.
Follow these steps to complete a CBD form:
- Open the OpenCampus app on your mobile device. From the app homepage menu, select the tasks tab.
- You will see a list of forms you are required to complete.
- Select the case based discussion form from my tasks and then click on start to open the form.
- Before handing your device over to the clinician (assessor) for feedback, enter a short description of the case discussed.
- Hand your device to the assessor for them to complete all relevant fields.
- The assessor will need to enter their name, role, email address and signature. They can then select submit. The form will then sync with the online system and update the status of the form to complete (green tick). The progress count for this portfolio activity will also be updated.
- You will be able to review the grades and feedback you received in the online system. On the portfolio page, select the case based discussion item underneath the relevant placement tab.
- Click on view next to the form you would like to review. This will open the completed form.
Your educational supervisor will review your forms and discuss this with you in your clinical placement review meeting
Clinical supervisor feedback forms should be completed in secondary care when ‘opportunistic’ or ‘real time’ feedback opportunities present themselves. Feedback can be received on any observed clinical activity.
Follow these steps to complete a clinical supervisor feedback form:
- Open the OpenCampus app on your mobile device. From the app homepage menu, select the tasks tab.
- You will see a list of forms you are required to complete. Select the clinical supervisor feedback form from my tasks and then click on start to open the form.
- Before handing your device over to the clinician (assessor) for feedback, enter the activity the clinician observed you performing.
- Hand your device over to the assessor. Your assessor will provide you with some feedback on your performance – the areas in which you perform well and areas where you can further improve.
- To complete the form the assessor will enter their name, role, email address and signature. They can then select submit. The form will then sync with the online system and update the status of the form to complete (green tick). The progress count for this portfolio activity will also be updated.
- You will be able to review the completed form in the online system. On the portfolio page select the clinical supervisor feedback item underneath the relevant placement tab.
- Click on view next to the form you would like to review. This will open the completed form.
Your educational supervisor will review your clinical supervisor feedback forms and discuss these with you in your clinical placement review meeting.
For each hospital based rotation (medicine or surgery), you must complete prescribing activities forms on the OpenCampus app. Prescribing activities include:
- Medicines Administration Round
- Pharmacy Dispensary Session
- Pharmacist Medication Review
Follow these steps to complete a prescribing activities form:
- Open the OpenCampus app on your mobile device. From the app homepage menu, select the tasks tab.
- You will see a list of forms you are required to complete. Select the prescribing activities form from my tasks and then click on start to open the form.
- Before handing your device over to the assessor (attending nurse or pharmacist), select the activity you’re getting signed-off.
- Hand your device to the assessor. Your assessor will enter their name and sign-off the activity. They can then select submit. The form will then sync with the online system and update the status of the form to complete (green tick). The progress count for this portfolio activity will also be updated.
- You will be able to review this form in the online system. On the portfolio page, select the prescribing activities item underneath the relevant placement tab.
- Click view next to the form you would like to review. This will open the completed form.
Your educational supervisor will review your prescribing activities forms and discuss these with you in your clinical placement review meeting.
