Guide overview

This article provides an overview of the MyProgress MB BS Phase III Assistantship forms and explains how to complete them.

The overarching aim of the Student Assistantship is to provide final year medical students with a realistic experience of clinical medicine in relation to what they will be doing when they start work as a Foundation Doctor in the August after their graduation.

Required Assistantship forms

You will need to complete the following forms.

Completion via the online MyProgress platform

One Self reflection form
One Reflection form

Completion via the MyProgress student app

One Induction form
Two Case based discussion forms
Three Team Assessment of Behaviour forms
One Review form

* You will also need to complete the Immediate Life Support course. Attendance at the Immediate Life Support course is compulsory. This is not a form in MyProgress. Your attendance will be recorded by the Hull York Medical School Programme Delivery team.

Assistantship forms to complete via the MyProgress online platform

Log into MyProgress via the online system

You can navigate to the MyProgress online platform to access an overview of all Assistantship forms, as well as review completed and submitted form content.

From a web browser navigate to the MyProgress online platform. Please access the following link for log in guidance; MyProgress log in guidance.
From the student homepage navigate to your Year 5 portfolio overview.
Select the Assistantship section.
Select the items tab to see all required Assistantship forms.

Assistantship: Self reflection form

Self reflection form

You need to complete one Self reflection form via the online MyProgress platform.

Select the complete new option from the Assistantship: Self reflection tab.
A new form window will open. There are several sections of the form that you need to complete; Developing Clinical Expertise, Reflection on Learning, Relationship with Patients, Relationship with Colleagues, and Personal Development.

In case of any connection issues we advise keeping a copy of your answers in a separate document outside of MyProgress.
If at this point you choose to save and close the form, you will be redirected back to the Assistantship: Self reflection tab. You can select complete new and choose to access the draft again.
You can delete a form in progress by selecting to delete in the top left-hand corner of the form.
If you are happy with your form, select submit in the top right-hand corner.
Confirm again. You will be redirected back to the Assistantship: Self reflection tab and will see a brief message stating the form was successfully submitted.
The completed form will be visible via completed responses under the Assistantship: Self reflection tab.
The tally of forms on the Assistantship: Self reflection tab will update with the number of completed form responses.

Assistantship: Reflection form

Reflection form

In the final week before the Assistantship review meeting, you must complete a reflection about your Assistantship experience. You are required to submit one Reflection form via the MyProgress online platform.

Select the complete new option from the Assistantship: Reflection tab.
A new form window will open. Complete the one required field to provide reflective feedback on your performance during the Assistantship period.
If you are happy with your form, select submit in the top right-hand corner.
Confirm again. You will be redirected back to the Assistantship: Reflection tab and will see a brief message stating the form was successfully submitted.
The completed form will be visible via completed responses under the Assistantship: Reflection tab.
The tally of forms on the Assistantship: Reflection tab will update with the number of completed form responses.

Assistantship forms to complete via the MyProgress app

Log into the MyProgress app

Log in to the MyProgress app. Please see the following link to log in guidance; MyProgess app log in guidance.
Select the Assistantship section from the MyProgress app student homepage.
You will see an overview of all Assistantship forms.

Forms will be deployed to your device at the start of the Assistantship period. Navigate to the MyProgress online platform to review completed content submitted via the app.

Assistantship: Induction form

Induction form

At the start of the Assistantship, you are required to meet with your clinical supervisor to discuss and agree objectives for the Assistantship period. During this meeting, you need to complete one Induction form.

This form is completed via the MyProgress app by yourself and your clinical supervisor.

Select the Assistantship: Induction form from the app homepage.
A new form will open. Select from the drop-down options to confirm you have had your induction with your clinical supervisor, that they have discussed the Assistantship checklist with you, and that you have agreed objectives for the Assistantship period.
Hand your device to your clinical supervisor. They can enter text into the comments field and will then need to sign the form and provide their name and NHS email address.
Click on submit to complete the form. Confirm submission again.
You will be redirected back to the app homepage. A brief green success message will appear to confirm submission.
The Assistantship: Induction form tab will update to reflect that the form has been completed.
Your clinical supervisor will receive an email confirmation with a link to a browser version of the completed form. The form can be reviewed but not edited further and does not require that they access the MyProgress online platform.
The completed form will be visible to you via completed responses under the Assistantship: Induction tab via the online portfolio platform.

Assistantship: Case based discussion form

Case based discussion form

During the Assistantship period, you are required to complete two Case based discussions (CBD).

These forms are completed on the MyProgress app and follow the same workflow as the CBDs you completed throughout Year 5 of the programme.

Select the Assistantship: Case based discussion form from the app homepage.
A new form will open. Enter the clinical setting in the text box provided. Then provide a case summary.
Hand your device to your assessor. They will choose grading from the drop-down options for medical record keeping, clinical assessment, investigation and referrals, immediate clinical management, follow up and future planning, professionalism, and overall clinical care. They can then enter agreed development into the text box field. They will then need to select their role from the drop-down options.
They will then need to sign the form and provide their name and NHS email address.
Click on submit to complete the form. Confirm submission again.
You will be redirected back to the app homepage. A brief green success message will appear to confirm submission.
The Assistantship: Case based discussion form tab will update to reflect the latest tally of completed forms.
Your assessor will receive an email confirmation with a link to a browser version of the completed form. The form can be reviewed but not edited further and does not require them to access the MyProgress online platform.
The completed form will be visible to you via completed responses under the Assistantship: Case based discussion form tab via the online portfolio platform.

Assistantship: Team Assessment of Behaviour form

Team Assessment of Behaviour form

You are required to complete a 360-degree Team Assessment of Behaviour (TAB) focusing on professional behaviours. During the second or third last week of your Assistantship, you must complete three TAB forms in the MyProgress app. One form should be completed by a supervising consultant, one by a foundation doctor, and one by a senior nurse (ideally Band 6 or 7) who has worked with you.

All three TAB forms should be completed before the Assistantship review meeting takes place.

Select the Assistantship: Team assessment of behaviour form from the app homepage.
A new form will open. Select the assessor position from the drop-down options.
Hand your device to your assessor. They can select from the drop-down options if they have any concerns regarding attitude/ behavior, team working with colleagues, and accessibility. If they have concerns they can provide more information in the text boxes provided. They can also provide additional comments, including positive feedback on performance.
They will then need to sign the form and provide their name and NHS email address.
Click on submit to complete the form. Confirm submission again.
You will be redirected back to the app homepage. A brief green success message will appear to confirm submission.
The Assistantship: Team assessment of behaviour tab will update to reflect the latest tally of completed forms.
Your assessor will receive an email confirmation with a link to a browser version of the completed form. The form can be reviewed but not edited further and does not require access to the MyProgress online platform.
The completed form will be visible to you via completed responses under the Assistantship: Team assessment of behaviour tab via the online portfolio platform.

Assistantship: Review form

Review form

In the final week of the Assistantship, you need to complete an Assistantship Review form with your clinical supervisor. This form requires sign-off from your supervisor via the MyProgress app.

Select the Assistantship: Review form from the app homepage.
A new form will open. Hand your device to your clinical supervisor.
They will then need to enter feedback in the areas you perform well and areas you can improve, and then select from the drop-down option if they have concerns or not.
They will then need to sign the form and provide their name and NHS email address.
Click on submit to complete the form. Confirm submission again.
You will be redirected back to the app homepage. A brief green success message will appear to confirm submission.
The Assistantship: Review form tab will update to reflect the latest tally of completed forms.
Your supervisor will receive an email confirmation with a link to a browser version of the completed form. The form can be reviewed but not edited further and does not require access to the MyProgress online platform.
The completed form will be visible to you via completed responses under the Assistantship: Review form tab via the online portfolio platform.
