MB BS: Phase II Portfolio Student Guide

Guide overview

This guide will explain how to complete the requirements of the Phase II portfolio in MyProgress.

MyProgress has replaced the OpenCampus platform, which was decommissioned in July 2025.

The MyProgress portfolio provides an ongoing record of your progress throughout the school. It is an important source of developmental (formative) feedback as well as a record of completed work and achieved grades. It should be the focus of a discussion with your Education Supervisor at the beginning and end of each new placement. Your supervisor will review all of your forms and discuss these with you in your Clinical Placement Review meeting.

You will need to work with your supervisors to ensure that all portfolio requirements are completed in a timely fashion. It is important to remember that satisfactory completion of the MyProgress portfolio is a mandatory requirement for entry into the end of year summative assessments (as outlined in the Code of Practice on Assessment and Examination).

For each placement in Phase II of the MB BS programme, you are required to complete a number of portfolio activities. Full details on the Phase II portfolio requirements can be found in the Phase II handbook available on Blackboard.

Online Portfolio forms

You will complete the following portfolio forms via the online platform.

Placement Allocation

Placement Allocation form workflow

The Placement Allocation form allows you to share your MyProgress portfolio with your Educational Supervisor. It’s important to complete this form at the start of each placement to ensure your supervisor has timely access.

Key information:

  • Completing the Placement Allocation form is the only way to provide supervisors with access to your portfolio.
  • You can grant access to multiple Educational Supervisors at any time during a placement.
  • Educational Supervisor access is automatically removed at the end of the placement.
  • Your supervisor will need to create an account on MyProgress, if they do not already have one. Instructions on how to do this can be found on our Educational Supervisor guide.

Follow these steps to complete the Placement Allocation form with your Educational Supervisor.

  1. Navigate to the portfolio forms for your current placement. Click on the items tab. Select complete new on the Placement Allocation tab. The placement allocation form will open in a pop-up window.

  2. Your supervisor needs to confirm they will act as your supervisor. They’ll then fill in their name and email address. The email address they enter will be used to create their account.

If your Educational Supervisor already has a MyProgress account, when completing this form they need to use the same email address linked to their account.

  1. In the top right-hand corner select Submit and then confirm the submission. You will be redirected back to the items tab and will see a brief message stating the form was successfully submitted.MyProgress success message form submission
  2. Your supervisor will get an email notification confirming that they completed the Placement Allocation form with you. Once logged in to MyProgress, they’ll be able to access your current placement portfolio forms directly from their homepage. If they don’t have a MyProgress account, they’ll receive an email with instructions to validate their account.
  3. The completed form will be visible via completed responses under the Placement Allocation tab. Only forms that have the status of submitted can be viewed under the Placement Allocation tab. Clerkings
Clerking form workflow

You are required to upload your completed clerking form (as a PDF or Word document) to the online MyProgress Clerking form.

Key information:

  • Use the template clerking forms available for download on Blackboard.
  • Please note: a different clerking form is used for the Mental Health block.
  • Before starting a clerking, you must make sure that you have obtained the patient’s consent. You must record this consent in the hospital or GP notes. You are also required to confirm consent was given in the online form.

The clerking form uses the email for later workflow. When you submit a clerking form, a link to the form will sent to your Educational Supervisor via email. Your Educational Supervisor can click on the link to provide you with a grade and feedback. It is important to submit your clerkings in advance of the placement deadline to ensure that your supervisor has sufficient time to provide you with feedback.

Follow these steps to complete a Clerking form in MyProgress:

  1. Download the Clerking form template from Blackboard. Complete the form and then save the form on your device.
  2. Navigate to your current placement portfolio forms in MyProgress. Click on the items tab. Select complete new on the Clerkings tab. A new window will open with a new form to complete.MyProgress Clerkings tab
  3. Complete all the student form fields as required. Input the relevant block, confirm patient consent, and upload your completed clerking form document – either via drag and drop or manual selection from your device.MyProgress clerking form
  4. If at this point you choose to save and close the form, you will be redirected back to the Clerkings tab. You can select complete new and choose to access the draft again.MyProgress draft clerking
  5. You can delete a form in progress by selecting to delete in the top left-hand corner of the form.
  6. If you are happy with your form, select email for later in the top right-hand corner to submit.MyProgress email for later option
  7. In the new window fill in the name and email address of the assessor.  You can disregard the message field and the option to flag the response. Select submit in the top right-hand corner and then confirm again you wish to send the form.MyProgress clerkings email form

  9. Only forms that have had both the student and supervisor parts completed can be viewed under the Clerkings tab. Access a record of a form awaiting supervisor approval via the Email option on the Clerkings tab.MyProgress pending clerking form
  10. MyProgress clerkings tallyThe supervisor will get an email link to the clerking form. From a browser they will complete the feedback, grade and  signature fields. The form will then close and no longer be accessible. The completed form will disappear from the email record and become visible via completed responses under the Clerkings tab. The tally of forms on the tab will update with the number of completed form responses.

Change the named supervisor or send a reminder email

  1. A record of an email for later form can be accessed via the email option on the Clerkings tab.MyProgress pending clerking email
  2. From the drop-down options next to an email record, change the supervisor or send a reminder email, (or delete a form).MyProgress send reminder clerking form
  3. Select send reminder to email the supervisor again.
  4. Select change recipient to send the form to a different supervisor. Fill in the name and email address of the supervisor in the new window.
  5. Select to send email. A ‘successfully changed recipient’ message will briefly appear.MyProgress clerkings email new supervisor form
  6. The new supervisor will get an email link to the clerking form, as described above.

Reflections
Reflections form workflow

Reflections must be completed using the online form. Full guidance on reflections—including expectations around length and quantity—can be found in the Ethics and Professionalism sections of the Year 3 and Year 4 courses on Blackboard.

Follow these steps to complete a Reflection form in MyProgress:

  1. Navigate to your current placement portfolio forms. Click on the items tab. Select complete new on the Reflections tab.MyProgress reflections tab
  2. In the new form window select the relevant block and enter your reflection in the text box.MyProgress reflections form
  3. If at this point you choose to save and close the form, you will be redirected back to the Reflections tab. You can select complete new and choose to access the draft again.

  4. You can delete a form in progress by selecting to delete in the top left-hand corner of the form.
  5. If you are happy with your form, select submit in the top right-hand corner. Confirm again. You will be redirected back to the Reflections tab and will see a brief message stating the form was successfully submitted.MyProgress success message form submission

Clinical Placement Review

The Clinical Placement Review form is completed by your Educational Supervisor during your review meeting at the end of each placement.  You should ensure your Clinical Placement Review meeting takes place before the end of your placement so your supervisor can complete the form on time.

This form will only appear under the Clinical Placement Review tab in your student view once it has been completed by your supervisor.

Follow these steps to review the Clinical Placement Review form completed by your Educational Supervisor:

  1. Navigate to your current placement portfolio forms. Click on the items tab. Select the Clinical Placement Review tab.MyProgress clinical placement review tab
  2. As stated above, you will have a view on the form once completed by your supervisor. When the completed form appears under responses, click on it to see detailed information.MyProgress clinical placement review meeting completed form

Portfolio forms completed via the mobile app

You will need to complete the following forms via the app.

OSLERs
OSLERs form workflow

At the start of an OSLER assessment, you will need to complete the first section of the OSLER form using the MyProgress app before handing your device to the assessor.

Assessor role:

  • The assessor will enter their feedback and grading directly into the app.
  • They will also provide their signature and email address.
  • MyProgress account is not required for the assessor to complete the form.
  • Once submitted, the assessor will receive an email confirmation of the completed form.

Follow these steps to complete an OSLER form in the MyProgress app:

  1. Select the OSLERs form from the portfolio forms section on the app homepage.Myprogress app OSLER form tab
  2. A new form will open.
  3. Select the relevant block from the drop-down options.Myprogress app OSLER form
  4. Hand your device to the assessor. Your assessor will provide you with a grade for the following criteria: gathering information, physical/mental state examination, problem solving and relationship with patients, patient management. They will also need to provide feedback in the feedback fields.
  5. They will then need to sign the form and provide their name and NHS email address. Click on submit to complete the form.

  8. The OSLERs tab will update to reflect the latest tally of completed forms.MyProgress app OSLERs tab
  9. The assessor will receive an email confirmation. The completed form will be visible to you via completed responses under the OSLERs tab via the online portfolio system.

You can view the assessor’s feedback by opening the form in the online system.

Case Based Discussions
Case Based Discussions form workflow

This form provides an opportunity to discuss a patient you’ve seen in secondary care and receive immediate feedback. Any qualified doctor in the hospital can facilitate this discussion.

You’ll need to complete the first section of the Case-Based Discussion form in the app. The assessor will then provide feedback and grading, enter their signature and email address, and submit the form. They do not need a MyProgress account. Once submitted, they’ll receive an email notification confirming the completed form.

Please do not submit a case based discussion for a patient on whom you have submitted a patient clerking.

Follow these steps to complete a Case Based Discussion form in the MyProgress app:

  1. Select the Case based discussions form from the portfolio forms section on the app homepage.Myprogress case based discussion app form.

  3. Enter a short description of the case discussed and select the relevant block from the drop-down options.Myprogress case based discussion app form
  4. Hand your device to the assessor. Your assessor will provide you with a grade for the following criteria: practising holistically, data gathering and interpretation, making a diagnosis/decisions, clinical management, maintaining an ethical approach and overall assessment. They may also provide feedback and recommendations for further development.
  5. They will then need to sign the form and provided their name and NHS email address. Click on submit to complete the form.Myprogress placement allocation app form

  8. The Case based discussions tab will update to reflect the latest tally of completed forms.
  9. The assessor will receive an email confirmation. The completed form will be visible to you via completed responses under the Case base discussions tab via the online portfolio system.Myprogress app Case based discussions tally

Clinical Supervisor Feedback

You are required to complete two Clinical Supervisor Feedback forms per placement:

  • One based on a patient consultation in primary care
  • One based on a physical examination in secondary care

After the observed activity, you’ll need to complete the first section of the form within the app. The assessor will then provide feedback, select their role, and enter their signature and NHS email address. Once submitted, they’ll receive an email notification confirming the completed form.

Follow these steps to complete a Clinical Supervisor Feedback form in the MyProgress app:

  4. Hand your device to the assessor. They will select their role and provide you feedback.
