MB BS: Completing Clinical Procedural Skills Forms

This guide provides a comprehensive overview of the Clinical Procedural Skills component of the MB BS portfolio. This guide is applicable to students in Phases II and III of the programme.

For detailed guidance on the procedural skills curriculum, please consult the Procedural Skills Handbook available from the procedural skills courses on Blackboard. It includes a list of required skills along with their respective deadlines. The handbook also provides clear instructions on completing forms, identifies approved assessors, outlines corrective actions, and offers skill-specific information. The Clinical Procedural Skills component is structured into categories for clarity and ease of use. These categories are as follows:

  • Teaching, SIMsafe and One-to-One forms
  • Formative forms
  • Summative forms
  • Revalidation forms (Year’s 4 and 5 only)

We will now look at each of these in more detail:

Teaching, SIMsafe and One-to-One Forms

Clinical Skills Facilitators are responsible for completing the Teaching, SIMsafe, and One-to-One forms. You can see that these forms have a label next to them which indicates they are available to other roles within the system.

This is where you can track your completed Teaching and SIMsafe activities.

All Formative, Summative and Revalidation procedural skills forms will be completed on the app in clinical practice whilst being observed by an appropriate assessor.

Formative Forms

Once you have attended the training and successfully completed the SIMsafe assessments, you will be able to undertake formative assessments in clinical practice. The Formative assessments aim to help you develop a skill whilst under the supervision of a qualified assessor. The assessor will give you detailed, descriptive feedback on your performance.

You must not practise SIMsafe skills on patients until you have successfully completed the SIMsafe assessment.  

Completing a Formative Assessment Form

To complete a Formative assessment form, follow these simple steps:

  1. Open the correct procedural skills form on the app.
  2. Select the placement site you wish to complete the form for (this will be where you’re currently based on placement). Click Confirm placement area to continue.
  3. Complete the first section of the form titled: To be completed by the student. You will need to: Indicate the ward, department or area where you completed the skill, agree to the SIMsafe declaration and enter the email address for the CSF team at your placement site.

It’s really important that you enter the correct email address of the CSF team based at your clinical placement site. The email addresses for each site can be found on the Clinical Skills forms.

A copy of the formative assessment you have completed will be sent to the Clinical Skills Facilitators.

  1. Once you have completed the top section of the form, hand your device to the assessor so they can provide you with feedback once they have observed you completing the skill.
  2. After reading the Formative Assessment requirements, the first thing the assessor must select is their role from the drop-down list.

Please note that some skills require the assessor to be specific about the procedural skill they observed. For example, the Administering Oxygen formative will ask them to select the delivery device demonstrated (Nasal cannulae, Non re-breathe mask, Simple face mask or Venturi system).

  1. Following this the assessor must complete the following sections of the form: a declaration that they observed you performing the skill, they must indicate if they gave you verbal feedback, they must provide written feedback on what you did well and what you could improve for next time and then they are required to sign the form.
  2. To complete the form, the assessor is required to enter their name and NHS email address.

Please note that Clinical Skills Facilitators will flag any forms submitted using a non-NHS email address. You maybe required to complete this form again.

Forms completed on the app with sync with the online system when your mobile device has an internet connection.

Summative Forms

You can only complete a summative assessment once you have completed the required number of formatives for that procedural skill. Make sure you check in the app or the online system that you have completed the formatives before undertaking the summative assessment.

To complete a Summative assessment form, follow these simple steps:

  1. Open the correct procedural skills form on the app.
  2. Select the placement site you wish to complete the form for (this will be where you’re currently based on placement). Click Confirm placement area to continue.
  3. Complete the first section of the form titled: To be completed by the student. You will need to: Indicate the ward, department or area where you completed the skillagree to the Formative declaration and enter the email address for the CSF team at your placement site.

It’s really important that you enter the correct email address of the CSF team based at your clinical placement site. The email addresses for each site can be found on the Clinical Skills forms.

A copy of the summative assessment you have completed will be sent to the Clinical Skills Facilitators.

  1. Once you have completed the top section of the form, hand your device to the assessor so they can provide you with feedback once they have observed you completing the skill.
  2. After reading the Summative Assessment requirements, the first thing the assessor must select is their role from the drop-down list.
  3. The assessor should then complete the following sections of the form: a declaration that they observed you performing the skill (includes select a Pass or Fail grade), they must indicate if they gave you verbal feedback, they must provide written feedback on what you did well, provide written feedback on what you could improve for next time, enter their registration number and then sign the form.

  1. To complete the form, the assessor is required to enter their name and NHS email address.

Revalidation Forms

In Years 4 and 5 of the MB BS programme, you will be required to complete Revalidation Forms for procedural skills previously undertaken in earlier years. The process for completing Revalidation Forms follows the same workflow as that used for Summative Forms.

To complete a Revalidation form, please refer to the instructions provided above under the Summative Assessment section.

Tracking your progress

You are able to track your progress against the procedural skills requirements in both the online system and the mobile app. The required number of completions for each procedural skill appears next to it in the list.

Here you can see the require number of Formatives for the Administering oxygen (2), Blood culture sampling (1) and Carry out continuous ECG monitoring skills (2). The student in this example has complete 1 of 2 required Administering oxygen forms.

Flagged Forms

The Clinical Skills Facilitators will ‘flag’ any procedural skills forms that do not meet the expected requirements.  The ‘Flagged Forms’ widget on your portfolio homepage will display any forms that have been flagged.

Clinical Skills Facilitators will provide a comment on any flagged forms, explaining the reason for the flag. They will also indicate whether the form needs to be resubmitted. Comments can be found at the bottom of each form.
